Close-out forms that need to be completed by all CTE teachers are as follows:

  • Inventory Form – Must include newly purchased items this school year provided by CTE or District.
  • Check-sheets – Used to record all students that participated in your program. 
  • ICTEP - One form completed for each special population student and attached to check-sheet.    
  • Textbook & Video Inventory – Must include all textbooks used for class instruction.  If using a software program please include that information.
  • Wish List or estimated budget form- Must be completed if you want equipment/materials for next year.
